Note: If registering 4 or
more attendees for the same course, you will receive a 15% discount on the
registration. Please indicate this in the comments field on the
form above or call the Process Improvement Institute at 1.865.675.3458. Discounts
for multiple registrations are available.
You may pay by credit card, purchase order,
check, or money order. If you issue a purchase order, we will invoice you
immediately and payment is due before the start of the course.

All course fees must be paid before the
course. Course fees include all manuals, course materials, lunches
(valued at $10.00 per day), and refreshments throughout the day. Fees do
not include accommodations or transportation. (Note: Please do not
make any nonrefundable travel arrangements until your course is
confirmed to be held. You will receive a confirmation of the course no
less than 2 weeks prior to the course; this will be sent by either
e-mail or fax [or both]).
